CPS Pharmacy Collaborative Practice Agreement Utilization - Baseline Survey Request

As a component of the collaborative grant between the Colorado Department of Public Health and Environment (CDPHE) and the Colorado Pharmacists Society (CPS), CPS is requesting information regarding Collaborative Practice Agreements (CPAs) utilized by pharmacies and pharmacy organizations throughout the state of Colorado.
CPS hopes to develop both a descriptive understanding of the extent of CPA use in Colorado, and also developed standardized CPA templates and a dedicated CPA repository. The primary aim is to offer assistance to pharmacies in establishing effective CPAs and to enhance pharmacy provider reimbursement by expanding the implementation of CPA agreements. By achieving these goals, we intend to strengthen pharmacy services and improve patient care in our State.
We are asking for your participation in completing the following survey. Your active involvement and participation in the survey will play a crucial role in helping us provide the necessary support and resources to all pharmacies in this endeavor. Thank you for your valuable contribution!
